Back when I was a trainee system admin we had a company come in to install a rack and fill it with new servers. They left the manuals kicking around and so I thought I'd read up on how it's done. It's then I realised they had installed the rack back to front before filling it. We'd paid extra for the installation.
Also, the first time I went on site at a new customer I was looking at upgrading their servers. I was messing around near the back of them and hit the UPS off switch by mistake. Everything was fine except the firmware was corrupt on one of the managed switches. Power cycling it is the only time you'd notice. They're still with us.
