Shao Kahn in Mortal Kombat classic series ( MK3, UMK3, MKT)

When I faced him for the first time, he was a nightmare to beat. He seemed damn fast and his hits were taking large chunks of my character life :mad:

and in MK9 when you fight him as Raiden, I nearly lost my tv through blunt force trauma by my ps3 controller!

Quake Wars in beta had some of the most OP weapons ever some by design others due to bugs

there was a build where the Anansi when it hit the ground would explode like a nuke killing everything within 1km in a huge single digit fps explosion and you could call in orbital laser (violator) strikes and hammer ICBM strikes with little restriction.

Most OP weapon in a game that I remember was some sword in the original diablo that set enemies on fire (area of affect), drained their health for several seconds on a hit and gave you that health, against a bigger enemy you were constantly at full health while dealing massive damage to anything nearby, combined with chain lightning even without the full skills for it and it was game over.
