Right handed, Left back pocket. Phone in front left pocket. Nothing in right front pocket as I use that to scratch things.

I ditched a big chunky wallet years ago and carry only my train pass and debit card with me in a slim holder... front right side.

If I take coins with me then they are in the front right coin pocket.

Phone front left.

I never put anything in the rear pockets. Things in there are too easy to steal.

Right handed.

It only feels right in the back left pocket. Plus it doesn't fit for comfort in any other pocket

No, it's just a short chain (about 8") that sits flush with my waistband, it's just long enough that I can comfortably hold my wallet. You can't really see it unless I have my wallet out. And no, no Velcro.
