Venom Playstation 2 Controller to PC adapter cable, which looks like this...

If so, how do you set it up so that both ports work?

One port works on mine, I went to play 2 player against my Brother on Pro Evo 6 and it didn't work.

Can someone help me!

Ive got a different make. But make sure ou disconnect it from the pc, plug the 2 controllers in, then plug it into the computer. Thats what I have to do with mine.

I have sorted it now
I had to go into the Pro Evo 6 settings on All Programs and under "Player 2" I had to tell it what to use from the Drop Down Box which in this case, was the second port on the adapter!
