Just follow the instructions in the app but in summary, yes download the My O2 app on the new phone, login and add the eSIM.
There's a transfer eSIM option in iOS settings which should do this for you but I am not sure if the backend...

How to fix:
Call your number from a different phone and leave a voicemail of several seconds in length
Wait for the voicemail to appear on your phone
Call the voicemail and listen to the message, do not delete it
Hang up
Call the voicemail again and listen to the message...
