If I had an indoor cat I'd probably take it out on a leash too so that if (when) it did escape the outdoors wouldn't be totally terrifying and it would know that the garden is safe and not to run away.
You said she works at a hospital? I assume that means NHS - get her to speak to HR or her manager about staff housing. Quality varies depending on Trust, but it's dirt cheap, and IIRC they take the rent out before tax (been a long time since I was in it though, so don't quote me on that)

Try calling him at dawn or dusk - if he's hiding locally he might respond then.
Have there been any builder or delivery vans around? There was a cat from Kent that found it's way to Hampshire in a builder's van. Though I guess most cats found like that are taken to the vets and scanned so...

I would suggest as soon as the vet says that he's got enough weight on for anaesthetic. You don't want him to spray at all - it reeks for days even if using bio-active agents to clean it.
Oscar-catte would appear dead on 8 o clock morning and evening for his meal; he'd try and push for an earlier evening meal, but never for a morning one.
Jess (3.5 kg) definately felt threatened by Oscar (4.5 kg and a year older) - she never really relaxed around him, and I eventually had to give him to a friend rather than have two unhappy cats.
